Many of our patients will require convalescent or hospice care and many have caregivers tending to their needs. As a result, we feel very strongly about helping caregivers with their needs. In previous blogs, we have discussed caregiver stress, depression, and burnout. Today, we will be discussing suicide.

Caregiving does not cause suicide, nor does it directly lead to it. However, caregivers are at increased risk for depression and burnout which can increase their risk factors for suicide. In fact, risk factors for caregiver depression and burnout, such as social isolation and serious physical and mental illness, are the same risk factors as for suicide.

More common than you think

Suicide is the sixth leading cause of death for all Americans and the 13th leading cause of death worldwide. On average, one suicide occurs every 16 minutes. According to Suicide.org, there are 750,000 suicide attempts and 33,000 suicide deaths each year.

Warning signs• Changes in eating and sleeping habits• Withdrawal from friends, family, and regular activities• Violent actions or rebellious behavior• Drug or alcohol abuse• Neglect of personal appearance• Marked personality change• Persistent boredom or difficulty concentrating• Loss of interest in once-pleasurable activities• Giving away favorite possessions, jewelry, pets, or money

What to do if you think a loved one is suicidal

Be direct. Ask the person directly if he or she is having suicidal thoughts, has a plan and has access to means. Asking someone if they have considered suicide will not put ideas into their head. Rather it will reduce their anxiety and give them an outlet. It will also give you information that indicates how strongly they have considered suicide.

Listen. Allow them to express their feelings without judgment. Accept their feelings. Their feelings are neither good nor bad, just feelings. Do not act shocked by their thoughts, this will only create emotional distance.

Look for red flags for suicidal behavior indicated by the phrase “IS PATH WARM?”• Ideation – Threatened or communicated?• Substance Abuse – Excessive or increased?• Purposeless – No reason for living?• Anxiety – Agitation/insomnia?• Trapped – Feeling there is no way out• Hopelessness• Withdrawing – From friends, family and society• Anger – Uncontrolled, rage, or seeking revenge• Recklessness – Risky acts, unthinking• Mood Changes

Take action. If you think the person might harm themselves, do not leave them alone. Remove any pills or guns. Recognize that you cannot single-handedly keep someone alive. Say, “I’m going to get you some help.” Then seek assistance from persons or agencies specializing in crisis intervention and suicide prevention.

What to do if you are thinking about suicide

Get help! Remember that suicide is final – once it’s done, there is no changing your mind. If you are feeling suicidal, you are in more pain than you know how to handle. There are ways to reduce the pain and better methods of dealing with it. You can learn both. Let someone help you.

im 7th grader girl who get straight A’s and a shelf full of trophies.but ever since my parents have been emotionally abusing me and always blaming me for everything my grades started dropping, im always sad, ive been getting in a lot of trouble, i would have suicidal thoughts, and i would always cry every time i see a parent who says “i love you” to their kid because i never heard my parents say “i love you” to me. my parents only say i love you to each other or to my sibling but never to me. my parents would call me stupid and i will never be successful in life because im stupid. they would say i wouldnt get a nice car or a nice house and ill work at mcdonald. whenever i study my parents would come and say “whats the point of studying, your going to fail the test”
there is more i would say but i would go on.
is this depression??

Question by Danielle: I have my first meeting with my doctor tomorrow for possible depression…what should I know?
I’m 15 years old, female, and my dad is on my case because “you haven’t had a bad life…has your mom ever been rolled out the door on a stretcher because she OD’d on Asprin trying to kill herself? NO!”. I’m really nervous.

What will the “tests” be like? Do my parents HAVE to be in the room with me/is there any way I can make them leave the room? When I do get diagnosed with it, how much will the medicine cost and what will the side-effects be? Will this make me do better in school? And finally, do I have to tell the doctor/my parents EVERYTHING…like suicidal thoughts, cutting, sexual activity, and such?

Whenever I even ask my dad for help with anything he answers then adds “Jesus…” to the end
For example: me “dad can you help me study for my test?” him “IM BUSY. jesus…”
I cry every night because of them, and i have had suicidal thoughts and thoughts about running away. i fell as if no one knows what im going through. please help

Normal moods are the body’s reaction to outside or inside stimuli, allowing everyone to handle, cope, or otherwise experience healthy emotions. Bipolar disorder, also called manic-depressive disorder, is one of the many mood disorders-abnormal psychological conditions characterized by extreme, repetitive swings in mood over prolonged periods in life.

Bipolar disorder is characterized by alternating episodes of mania (high energy and elated thought) and depression (low energy and blues). In some cases, however, symptoms of mania and depression may occur together in a state called Mixed Bipolar State. Bipolar disorder facts show that over 2 million adult Americans suffer every year from it.

Types of Bipolar Disorder

Two main types of bipolar disorder are commonly known: bipolar I disorder and bipolar II disorder. In the former case, the patient has experienced at least one episode of high mania. Numerous episodes of hypomania alternating with depression are called cyclothymic disorder.

Highs

Sufferers of bipolar mood disorder experience the presence of highs, which are characterized by increased physical and mental activity and energy; heightened mood, exaggerated optimism and self-confidence; excessive irritability, aggressive behavior; decreased need for sleep without experiencing fatigue; grandiose delusions; racing speech; impulsiveness; reckless behavior; delusions and hallucinations.

Lows

Bipolar mood disorder, as well as other depressive disorders include severe lows, such as prolonged sadness or unexplained crying spells; significant changes in sleep patterns; irritability, anger, worry, agitation; pessimism; loss of energy or extreme fatigue; feelings of guilt or worthlessness; inability to concentrate, unexplained aches and pains, and suicidal thoughts.

Manic Episodes

Manic episodes are characterized by high activity, irritability and restlessness, euphoric feeling, racing thinking and speech hopping from topic to topic, impulsiveness, lack of concentration and poor judgment. In many cases the manic-depressive shows erratic or rash behavior e.g. in driving or sexual activity. In severe cases of mania, hallucinations (hearing voices that are not there) and delusions (strong irrational beliefs about reality) may also occur.

Depressive Episodes

Depression includes gloomy mood with feelings of emptiness and often crying spells. Pessimism, indifference, indecisiveness, and (in extreme cases) recurring thoughts of death and suicide are other main indicators of depression.

Victims of Bipolar Disorder

According to published bipolar disorder facts, the disorder occurs in all ages, races, ethnic groups and social classes. Men and women equally suffer from it, though women have been reported to experience more rapid cycling of mood swings.

The Age of Vulnerability

Bipolar disorder facts reveal that this abnormality mostly strikes in late adolescence or early adult life (90 % cases start before age 20).

Diagnosis

Patients thought to be suffering from bipolar mood disorder cannot be self-diagnosed. Since there are no lab tests that can indicate the presence of the disorder, a trained medical or mental health professional must conduct interviews and tests; if medication is required, it must be strictly managed.
